Thank you for your interest in registering! We will be notified as soon as you have registered via the website. We will then register you within a few days or you will be put on the waiting list. When the registration has been processed, you will receive a confirmation email (also keep an eye on your spam box!). From that moment on we are your doctor. Then do not forget to unsubscribe from your old GP so that we can receive your medical file.

  1. Complete the registration form on the website.
  2. You will receive an email when we have registered you (from that moment on we are your GP).
  3. You request your old GP to send your medical file to us.

Register

Do you live within 15 minutes cycling of our GP practice? Then you can register with us as a patient.

If you also want to register your partner and/or child(ren), please state their name, date of birth, phone number and BSN in the ‘Comment for doctor’ field.

As soon as we have processed your registration, you will receive a confirmation e-mail. This can take several days.

IMPORTANT: Please inform your previous GP practice that you’ve moved to us. This way, we will receive your medical file sooner, which enables us to help you better.

Practical Information

All consultations by patients with a Dutch health insurance are invoiced directly to the insurer. You can find the rates on the website of the Dutch Healthcare Authority, the NZa.

Our practice is accredited. This means that you can trust that the practice is well organised. It also means that the practice continues to develop in order to be able to help you better and better. Read more here (in Dutch).
